REGISTRATION FORMS ARE AVAILABLE ONLINE AT www.sjbparish.com OR PICKED UP AND MUST BE RETURNED BY MAIL OR IN PERSON TO THE RECREATION DEPARTMENT LOCATED AT 1801 W. AIRLINE HWY IN LAPLACE AND A FEE OF $40.00 IS REQUIRED.
